• Submit a completed application form (located below).

• Submit a $60.00 nonrefundable application fee payable to Chamberlain College of Nursing.

• Request official transcript documenting graduation from a state approved high school or successful completion of the General Education Development Examination (GED).

• Request official transcripts from all post-secondary schools attended to be sent directly to Chamberlain College of Nursing.

• Submit a photocopy of unrestricted nursing license (if applicable).

• Request a copy of ACT or SAT scores to be sent directly to the College; Chamberlain College of Nursing’s school code for the ACT is 2293 and for the SAT is 3139.

• Submit the statement of eligibility, properly signed and dated, concerning the State Board of Nursing Examination (NCLEX), found on the back of the application.

• International students must submit an official transcript evaluation from a NACES approved evaluating agency.

• Applicants whose native language is not English must complete the Test of English as a Foreign Language (TOEFL). Admission documents may be mailed or faxed to the address indicated on the application.
