- Submit a $60.00 nonrefundable application fee payable to Chamberlain College of Nursing.
- Request official transcript documenting graduation from a state approved high school or successful completion of the General Education Development Examination (GED).
- Request official transcripts from all post-secondary schools attended to be sent directly to Chamberlain College of Nursing.
- Submit a photocopy of unrestricted nursing license (if applicable).
- Request a copy of ACT or SAT scores to be sent directly to the College; Chamberlain College of Nursing’s school code for the ACT is 2293 and for the SAT is 3139.
- Submit the statement of eligibility, properly signed and dated, concerning the State Board of Nursing Examination (NCLEX), found on the back of the application.
- International students must submit an official transcript evaluation from a NACES approved evaluating agency.
- Applicants whose native language is not English must complete the Test of English as a Foreign Language (TOEFL). Admission documents may be mailed or faxed to the address indicated on the application.
