For state license applications and verifications (includes pharmacology or clinical hours) please complete the Student Release Form. Email your completed form to CCNVerifications@chamberlain.edu.

 

Request Transcripts

Chamberlain University transcripts can be requested online or via the student portal at community.chamberlain.edu. Electronic transcripts are $10 each and paper transcripts are $15 each. Each student will receive one complimentary official transcript upon conferral*.

Go to the Transcripts request form and complete to order your transcripts online.
