Your Official Transcripts
Chamberlain requires all applicants to submit official transcripts sent directly from your previous educational institution(s) to Chamberlain College of Nursing.
Our admissions team is happy to request them, and Chamberlain will incur the expense associated with requesting these official transcripts. All we need from you is authorization to obtain the official transcripts. An admission representative will send you an electronic transcript request form for this purpose.

Social Security Number Requirement
Please note that your SSN is required on the NursingCAS application.
Chamberlain College of Nursing is required to collect the social security number (SSN) for a variety of legally mandated activities, including income tax reporting and administration of federally supported financial aid programs. The SSN is not used as the student’s primary identification; however, students who do not provide this information at the point of application may experience a delay in financial aid processing. Chamberlain College of Nursing has adopted privacy policies and practices designed to protect student’s personal information. Only information required to efficiently conduct our business and meet state and federal reporting requirements is collected. The information collected is only disclosed as permitted under the Family Educational Rights and Privacy Act of 1974 as amended (FERPA). The FERPA policy is printed in the student handbook and is available for review in the Student Services office.
A2 Admission Assessment Fee
Pre-licensure applicants to Chamberlain’s BSN or ADN degree programs are not required to pay a fee for the A2 Admission Assessment unless the applicant chooses to test or retest beyond 30 days of submitting an application for admission. If it is necessary to reschedule your exam, a Chamberlain admission representative will assist you. The A2 Admission Assessment can be completed at any of Chamberlain College of Nursing's campus locations.
Only one application fee is required to apply to Chamberlain College of Nursing. For AACN NursingCAS applicants, Chamberlain may request an e-signed application fee waiver form.
If you have previously applied through and made payment to Chamberlain College of Nursing, note that your AACN NursingCAS application to Chamberlain will also require an application fee. Chamberlain College of Nursing cannot refund fees submitted through AACN NursingCAS but will refund a duplicate application fee paid directly to Chamberlain when made within the past 10 days.
If you have inadvertently submitted payment to Chamberlain College of Nursing after having submitted your application fee to AACN, please contact Chamberlain at [email protected] within 10 days of payment and your Chamberlain application fee will be refunded.
